Admitting and Registration

Admission and Registration is arranged through your physician.

When you arrive at the hospital, go to the Admitting Office Desk, located in the front lobby. Bring your Social Security number, Insurance card/ Medicare/MediCal/Group insurance with address, & Advance Directive. Be sure you bring your insurance company's address with you.

Patients requesting financial assistance may contact the hospital Financial Counseling office at (530) 926-7245.  The integrity of clinical decision making is not affected by compensation arrangements.  That means the decision to receive treatment or to remain in the hospital is based on medical necessity, not on payment arrangements. 

An admission form must be signed to authorized the medical staff and hospital personnel to provide you with care.  Your signature may be required on addition consent forms.  If you are under the age of consent, a parent or legal guardian must sign for you.  Legal guardian documentation is required. 

Federal regulations require the hospital to ask you about advance medical directives that allow you to give instructions about your future medical care.  A Durable Power of Attorney for Health Care designates someone you select to represent your wishes. It is your right to accept or refuse medical care.  Advance Directives protect your rights if you become mentally or physically unable to communicate your wishes due to an injury or illness.  An information brochure and forms are available at the Admitting Office.

For protection, all patients are identified by a plastic wrist band containing necessary information.  Please, do not tamper or remove it during your stay.
